Heartland Hospice is seeking a registered nurse to lead hospice and palliative care in patients’ homes in the Apex area. You will conduct comprehensive assessments, implement and adjust plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, patient-centered care.

You will educate families, manage symptoms, document progress, and participate in on-call coverage, all while maintaining regulatory compliance and high standards of care.

Formación

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsabilidades

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans with visit frequency and risk assessment.
• Coordinate care with physicians and the IDG.
• Educate patients and caregivers on disease processes, self-care, medications, and end-of-life care.
• Communicate changes in condition to the care team for timely interventions.
• Document clinical records timely and maintain infection control standards.
• Support care continuity across home health, hospice, and transitions in care.
• Participate in on-call and patient support activities as applicable.
