Job summary

Iowa City Hospice is seeking a full-time Hospice RN Case Manager to coordinate and oversee patient-centered care across homes, assisted living, and long-term care settings. You’ll lead the interdisciplinary team, manage plans of care, and advocate for patients and families with clinical expertise and compassion.

The role emphasizes autonomy, high-quality end-of-life care, and collaboration with physicians, social workers, and caregivers to promote comfort and dignity.

Qualifications

• RN license in Iowa or compact license
• Two years of home health or hospice experience preferred
• Excellent communication, organization, and critical thinking skills

Responsibilities

• Coordinate and oversee the patient’s plan of care as the primary RN Case Manager.
• Complete nursing assessments and ongoing evaluations of patient needs.
• Develop and update individualized care plans based on goals and changes.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Provide skilled nursing care in home and facility settings.
