Job summary

Heartland Hospice seeks an experienced Registered Nurse to join our team as a Hospice RN / RN Case Manager. You will lead patient assessments, implement care plans, and work closely with physicians and social workers to support terminally ill patients and their families in the home setting.

You will educate families, coordinate care across the IDG, and document therapies and outcomes while meeting regulatory standards and delivering compassionate, dignified end‑of‑life care.

Qualifications

• Current license to practice nursing in the state of requested employment.
• Minimum of 1 year experience as an RN in hospice, palliative care, home health, or related areas.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with IDG and other providers.
• Educate patients and caregivers on disease, care plans, and end‑of‑life options.
• Document care accurately and maintain confidentiality.
