Advanced Health Care is seeking a compassionate Hospice RN Case Manager to join our team in Mesa, AZ. You will oversee patient-centered end-of-life plans, coordinate with an interdisciplinary team, and support families through challenging times.

Qualified candidates are licensed RNs with hospice experience, strong clinical skills, and a commitment to patient dignity. The role includes weekdays hours with on-call shifts and requires reliable transportation for home visits.

Qualifications

• Current RN license in the state of practice.
• 1 year of hospice or related healthcare experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of hospice care regulations and documentation.
• Experience with electronic medical record systems.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including pain and symptom management.
• Coordinate and supervise hospice aides and healthcare professionals.
• Communicate with patients, families, and team about progress and plans.
• Monitor physical, emotional, and spiritual well-being.
• Collaborate with interdisciplinary team to align care with patient goals.
• Ensure regulatory compliance and proper documentation.
• Provide bereavement support to families as needed.
• Participate in team meetings to discuss care plans and barriers.
• Perform other duties assigned by supervisor.
