St. Croix Hospice in Oakdale, MN seeks an RN Case Manager to lead the hospice care team, develop and manage each patient’s Plan of Care in collaboration with the Hospice Medical Director, patients, families, and facility staff.

The RN Case Manager provides day-to-day patient care, manages treatments, maintains records, coordinates with physicians and interdisciplinary teams, and supports activities of daily living with a focus on comfort and quality of life.

Qualifikationen

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Nurses with a multistate license can practice in other states without additional licenses while maintaining RN license in primary state.
• Ability to pass DHS background check, consumer report, and drug test.
• Valid State Driver’s License and access to reliable automobile.

Aufgaben

• Provides day-to-day patient care and administers medications as prescribed.
• Leads and coordinates care with physicians, interdisciplinary teams, and community partners.
• Maintains accurate clinical documentation including pain management and symptom changes.
• Develops and revises the Plan of Care based on patient evaluations.
• Educates patients and families on self-care, medications, and diet as appropriate.
• Arranges for medical equipment and supplies and ensures HIPAA compliance.
