Job summary

St. Croix Hospice in Minnesota is seeking a Registered Nurse Case Manager to lead the patient’s hospice care team and develop the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RNCM provides day-to-day patient care, administers medications, maintains records, coordinates with the care team, and ensures HIPAA compliance while focusing on comfort and quality of life for patients and their families.

Qualifications

• Must have a current RN license in state(s) of practice.
• Multistate licensure accepted; PSOR considerations apply.
• Graduate of a professional school of nursing and licensed in applicable states.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and reliable vehicle required.

Responsibilities

• Provides day‑to‑day patient care in hospice settings.
• Develops, implements, and revises the Plan of Care.
• Administers medications and treatments as prescribed.
• Coordinates care with physicians, teams, and partners.
• Ensures HIPAA compliance and accurate documentation.
• Educates patients and families on care plans and self‑care.
