Job summary

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team in the St. Cloud area, with responsibilities extending to Willmar & Litchfield.

You will develop and manage individual Plans of Care in collaboration with physicians, patients, and families. You will provide direct patient care, administer medications, document clinical notes, coordinate with interdisciplinary teams, and support families to improve quality of life in a compassionate

Qualifications

• Current RN license in state of practice.
• Ability to pass DHS background check and drug test.
• Experience in hospice/home care preferred.

Responsibilities

• Lead the patient’s hospice care team and develop Plans of Care.
• Administer medications and treatments per physician plan.
• Maintain accurate clinical documentation and records.
• Coordinate care with physicians and interdisciplinary teams.
• Educate patients and families on self-care and care plans.
• Participate in on-call visits and attend group meetings.
