Job summary

The RN Case Manager – Care and Comfort in Tallahassee, FL develops and coordinates the interdisciplinary plan of care for hospice patients to ensure quality of life. The role includes symptom management, patient/family education, and timely assessments with documentation in EMR.

Supervises Hospice Aide and LPN while collaborating with IDT and physicians. Minimum one year inpatient nursing or hospice/home health experience is required with a Florida RN license; travel within the area is expected

Qualifications

• Active Florida RN license required.
• Minimum of one year inpatient nursing or hospice/home health experience.
• Education level matches: 4 Year Degree or higher.

Responsibilities

• Coordinate and implement interdisciplinary care plans for hospice patients.
• Provide symptom management and patient/family education.
• Complete timely patient assessments and EMR documentation.
• Supervise Hospice Aide and LPN per schedule.
• Collaborate with IDT members and attending physician.
• Support quality improvement activities and regulatory compliance.
• Maintain professional communication with patients, families, and facilities.
