Job summary

Community Hospice & Palliative Care in Fernandina Beach, FL is seeking an RN Case Manager to plan and manage nursing care for a caseload in home or long-term care settings, coordinating with the interdisciplinary team to achieve patient- and family-directed goals.

The role requires Florida RN licensure, CPR certification, a valid driver's license, ADN or BSN, minimum one year of RN experience, and ability to work flexible hours.

Qualifications

• Current Registered Nurse license in Florida.
• CPR certification.
• Valid driver's license and clean driving record.
• ADN or BSN from accredited program.
• Minimum one year RN experience; hospital med-surgical/oncology preferred.

Responsibilities

• Assess patient and family goals; align with care plans.
• Coordinate care across care settings and with IDG meetings.
• Provide education to patients and caregivers about disease progression and hospice philosophy.
• Maintain accurate, timely documentation.
• Support team members and participate in quality improvement.
• Respond to urgent patient needs promptly.
