Job summary

Community Hospice & Palliative Care in Florida is seeking an experienced RN Case Manager to plan and manage nursing care for an assigned caseload in home or long-term care settings. In collaboration with the interdisciplinary team, you ensure patient- and family-directed goals are achieved using a holistic approach, provide education, coordinate care across sites, and adapt to varying work locations.

This role requires an active Florida RN license, CPR certification, and a degree (ADN/BSN).

Qualifications

Responsibilities

• Assess and identify patient and family goals and needs.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Coordinate timely and effective responses to changing or urgent patient needs.
• Maintain accurate, timely, and compliant documentation.
• Support and educate team members, patients, families, and the broader community.
• Participate in internal quality improvement initiatives.
• Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
• Perform other duties as assigned.
