Job summary

Affinity Health Management is seeking a Registered Nurse to provide home health and hospice care in the Florida region. The RN will assess patients, develop and implement care plans, and coordinate with physicians and the inter-disciplinary team to support patient goals.

The role requires a Florida RN license, excellent communication, and the ability to work in a home-care setting with weekend on-call duties as needed.

Qualifications

• Requires a nursing degree and Florida RN license.
• Minimum 1 year of professional nursing experience; public health/home/hospice preferred.

Responsibilities

• Perform comprehensive patient and family assessments and develop plans of care.
• Provide professional nursing care and administer prescribed treatments.
• Document patient status and care notes; coordinate with interdisciplinary team.
• Educate patients and families; ensure safety and continuity of care.
