Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities in Florida. You will apply nursing knowledge to assess needs, develop care plans, and collaborate with physicians and families to improve comfort and quality of life.

The role requires on-call duties, driving to patient homes, documenting care, and coordinating with the hospice interdisciplinary team.

Qualifications

Graduate of an accredited school of nursing.
Florida RN license in good standing.
One year of professional nursing experience within the last three years preferred in home health/hospice.
Excellent observation, communication and problem-solving skills.

Responsibilities

Perform comprehensive patient and family assessments to determine hospice needs.
Develop and implement care plans; initiate and adjust as patient needs change.
Provide professional nursing care and administer prescribed treatments.
Maintain accurate clinical notes and communicate with physicians and care teams.
Coordinate with family and interdisciplinary group to meet emotional and care needs.
Participate in on-call duties and weekend coverage.
