Job summary

Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, coordinate, and deliver hospice care in home settings throughout Bristol, VA. This full-time role emphasizes community health education and family support.

You will lead the interdisciplinary team, develop and document the patient care plan, and ensure high-quality documentation and on-call coverage. A valid RN license, CPR, and driving are required.

Qualifications

• RN license active in Virginia with CPR certification.
• Minimum 2 years RN experience in clinical care, home health, or hospice.
• Must have valid driver's license and insured vehicle.
• Excellent observation and communication skills; nursing skills per competency checklist.
• Ability to lift, move, and position patients and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop, and document the patient plan of care.
• Provide care with OSHA infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Perform on-call responsibilities and assist patients/families as assigned.
• Supervise hospice aides and licensed practical nurses.
