Job summary

Affinity Hospice in Bristol, VA is seeking a compassionate Registered Nurse Case Manager to join our growing team. This full-time role plans, organizes, and directs hospice care for patients in their homes and communities, coordinating care with the interdisciplinary team.

The ideal candidate has an active RN license, CPR certification, and at least two years of nursing experience in home health or hospice settings; valid driver's license and reliable vehicle required.

Qualifications

• RN license active and in good standing.
• Minimum two years of RN experience in clinical care, home health, or hospice.
• Ability to drive and operate a vehicle in accordance with state requirements.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures to protect staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Perform on-call responsibilities as assigned.
• Supervise hospice aides and licensed practical nurses.
