Responsibilities

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Assess physical, emotional, and spiritual needs throughout the care journey
• Monitor vital signs and document changes promptly
• Collaborate with interdisciplinary teams to ensure comprehensive care
