Job summary

Gentiva is seeking a Registered Nurse Case Manager to lead in-person and telephonic nursing care for hospice patients in home settings. You will perform assessments, implement plans of care and coordinate with an interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Requires RN licensure and at least 1 year of RN experience, with hospice or home health experience preferred. Benefits include comprehensive coverage and on-call support.

Qualifications

• Active RN license in the state of employment
• Hospice or palliative care experience preferred
• Minimum of 1 year RN experience in acute care, hospice or home health

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or patient settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams including physicians and nurses.
• Educate patients, families and caregivers on disease processes, end-of-life care, medications and nutrition.
