Job summary

Gentiva Hospice is seeking an RN to provide comprehensive hospice and palliative care in patients' homes or related facilities. You will conduct assessments, implement the Plan of Care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, individualized care.

Today’s role emphasizes strong communication, documentation, and adherence to regulatory standards while supporting families through end-of-life decisions and education.

Qualifications

• Registered Nurse with required licensure to practice in the state.
• Minimum of 1 year RN experience in acute or hospice settings preferred.
• Experience in hospice or palliative care is highly desirable.

Responsibilities

• Be the primary contact for patients and families in hospice planning.
• Deliver and document skilled nursing care per Plan of Care, in home or facility.
• Perform initial/ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
