Job summary

LUMINOUS Hospice is seeking an experienced RN Case Manager to join our care team in Tulsa, OK. You will collaborate with the Clinical Director and a multidisciplinary group to deliver compassionate, patient-centered home hospice services.

You will coordinate care plans, educate families, monitor patient progress, and ensure timely documentation. Current RN license, valid driver's license, and CPR certification required; hospice experience preferred.

Qualifications

• Current State license as a Registered Nurse.
• Current driver's license.
• Current CPR Certification.
• Two (2) years' RN experience in clinical care; hospice preferred.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.
