Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in home and community settings. The role emphasizes clinical nursing expertise and coordination of services with families and the interdisciplinary team.

The ideal candidate holds an active RN license in Texas, CPR certification, and preferably BSN with hospice or home health experience. Strong assessment, planning, and communication skills are essential for delivering compassionate end-of-life

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Management experience is not required; may supervise hospice aides.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family.
• Develops and revises plan of care with nursing diagnoses.
• Administers medications and treatments as prescribed.
• Coordinates patient care with the interdisciplinary team.
• Performs on-call duties as defined by policy.
