Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize and direct hospice care in patients’ homes and communities. The role emphasizes nursing expertise, community health experience, and collaborating with an interdisciplinary team to meet patient needs with dignity and comfort.

The RN Case Manager builds on community resources to tailor care plans, coordinating treatments and educating families while maintaining compassionate, high-quality care.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, hospice care experience preferred.
• Management experience is not required.

Responsibilities

• Complete initial and ongoing assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family input.
• Coordinate patient care as Case Manager with the interdisciplinary team.
