Job summary

Luminos Hospice is seeking an RN Case Manager to lead a care team (LPNs, LVNs, Aides, MSW, Chaplain) and provide best-in-class hospice care in patients' homes. You will collaborate with the Clinical Director to coordinate care and ensure quality outcomes.

Requirements include current OK RN license, CPR, and a couple of years of RN experience in a clinical setting; hospice experience preferred. Travel and driving are required.

Qualifications

• Current State license as RN (OK).
• Current driver’s license.
• Current CPR certification.
• Two years of RN experience in clinical care; hospice experience preferred.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head-to-toe physical assessments.
• Monitors vital signs and manages medications.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises HHAs.
• Communicates patient progress to clinical manager and team.
• Other duties as assigned.
