Job summary

Luminary Hospice in Texas is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice services. You will bring your nursing expertise to patients in their homes and communities, coordinating care with physicians and an interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing with RN licensure in Texas.
• Current CPR certification required.
• 1–2 years in acute care and hospice/home health experience preferred.
• Excellent observation, communication, problem-solving and math skills.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care following the nursing process and physician plan of care.
• Develop and revise care plans with patient/family participation.
• Administer medications and treatments as prescribed; educate on self-care and disease management.
• Coordinate with physician and interdisciplinary team to ensure continuity of care.
• Participate in on-call duties and quality improvement initiatives.
