Job summary

Qualifications

• Graduate of an accredited school of nursing with active state licensure and CPR certification.
• Bachelor’s degree; hospice care experience preferred.
• Experience supervising hospice aides is not required.
• Strong observation, verbal and written communication, problem-solving, and math skills.

Responsibilities

• Perform comprehensive initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and administer prescribed treatments.
• Develop and adjust care plans with patient and family input; reassess needs regularly.
• Coordinate with physicians and interdisciplinary team; ensure documentation is timely and accurate.
• Complete and maintain clinical notes; communicate changes in patient condition.
• Participate in on-call duties and ensure available equipment and services.
