Job summary

Luminos Hospice is seeking an experienced RN Case Manager to lead a care team and coordinate home-based hospice services with the Clinical Director. You will develop and monitor patient-centered care plans to ensure exceptional end-of-life care.

Required: active RN license, CPR, valid driver’s license, and two years’ RN experience in clinical care; hospice experience preferred. This full-time, on-site role includes comprehensive benefits and PTO.

Qualifications

• Nursing skills and judgment per standard practice.
• Knowledge of state and federal healthcare regulations.
• Ability to work independently and make sound clinical decisions.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager and care team.
• Other duties as assigned.
