Job summary

Luminos Hospice is hiring an RN Case Manager to lead a care team delivering exceptional end‑of‑life home care. You will work with the Clinical Director to coordinate patient plans and mentor LPNs, LVNs, aides, MSW, and a Chaplain.

Requirements include current RN license, driver’s license, CPR, and hospice experience is preferred. Strong clinical judgment and communication skills are essential for successful patient outcomes.

Qualifications

• Current State license as a Registered Nurse (RN).
• Current driver’s license.
• Current CPR Certification.
• Two (2) years’ RN experience in clinical care; hospice experience preferred.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.
