Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home settings and affiliated facilities. You will assess, implement, and adjust plans of care, coordinating with an interdisciplinary team to ensure compassionate, patient-centered care.

Responsibilities include symptom management, education for caregivers, and maintaining timely clinical documentation while complying with regulations. A valid RN license and CPR are required.

Qualifications

• Minimum of 1 year RN experience in hospice, palliative care, or home health.
• Current license to practice as an RN in the state of employment.
• CPR certification.
• Ability to operate nursing assessment tools and maintain documentation.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per the Plan of Care.
• Develop and update individualized care plans with appropriate visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Provide education and bereavement support to patients and families.
