Job summary

Gentiva is seeking a Hospice RN Case Manager to lead in-home patient care, assessments, and interdisciplinary collaboration. Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Strong communication and teaching skills for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver and document skilled nursing care in patients' homes or facilities.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with IDG and other clinicians.
• Educate patients, families, and caregivers about disease, care goals, and end-of-life planning.
• Maintain accurate clinical documentation and ensure regulatory compliance.
