Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through end-of-life care in home settings. You will assess, plan, and implement personalized care plans with an interdisciplinary team, educate families, and ensure compassionate, compliant nursing support.

Responsibilities include conducting assessments, managing symptoms, coordinating visits, and maintaining accurate documentation while participating in on-call coverage as needed.

Qualifications

• RN licensure in the state of employment
• Minimum 1 year of RN experience in acute care, hospice or home health
• CPR certification
• Current automobile insurance and valid driver’s license optional in some regions

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care with visit frequency
• Coordinate care across interdisciplinary teams including physicians and other care providers
• Provide patient and caregiver education on disease process, end-of-life care, medications, nutrition and self-care
• Document timely clinical records and maintain infection control standards
• Support continuity of care with referrals and transitions between levels of care
• Participate in on-call and patient support activities as applicable
