Job summary

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital setting preferred.
• Active BLS certification.
• Ability to visit patients in their homes and use EMR (Electronic Medical/Health Record).
• Valid driver’s license and automobile insurance per company policy.

Responsibilities

• Provide direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinate the Plan of Care to reflect the patient’s evolving needs.
• Educate patient and family about disease, palliation, care giving and safety.
