Job summary

Chapters Health System, Inc. is hiring an RN Case Manager to assess patient and family needs, coordinate care with the IDT, and provide clinical, palliative, and supportive care in patients’ homes to keep them in their home environment as long as possible.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and auto insurance per company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.
• Ability to perform essential functions including lifting, bending, kneeling with/without accommodation.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, prevention, palliative care, and safety practices.
• Presents concise reports to IDT and collaborates across disciplines.
• Initiates skilled nursing interventions to alleviate symptoms and improve comfort.
• Maintains thorough documentation in the EMR.
• Communicates with physicians, staff, patients and families; SBAR as needed.
• Supervises care provided by Home Health Aides as requested.
