Job summary

Chapters Health System is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to enable patients to remain at home whenever possible.

The role requires coordinating care across IDT, ensuring timely physician orders, and supporting patients and families through education and compassionate communication.

Qualifications

• RN license current in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital.
• Alzheimer's Disease and Related Dementias Training (Florida) certification if employed at PACE.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, palliative care, safety and care planning.
• Performs home visits to assess safety, medication and nutritional needs.
• Documents changes and communicates with IDT; initiates Plan of Care.
