Chapters Health System is seeking a registered nurse case manager to coordinate care for patients in home-based settings. You will assess patient needs, plan care with the Interdisciplinary Team, and deliver supportive, clinical services to keep patients safely in their homes as long as possible.

Role requires RN licensure, at least 1 year of hospice or hospital nursing experience, and proficiency with EMR/EHR systems. Travel to patient homes and driving responsibilities are expected.

Formación

• Current RN license in the state where employed.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience using EMR/EHR systems.

Responsabilidades

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patients, families, and caregivers about disease, palliative care and safety.
• Home visits to assess safety, medication and adherence.
• Reports changes in patient condition to IDT and physicians.
• Participates with the IDT to evaluate hospice referrals/admissions.
• Presents concise reports to IDT; notes input from all disciplines.
• Communicates clearly with physicians, staff, patients and families; documents interactions in EMR.
• Supervises Home Health Aides as requested.
• During emergencies, may be required to report to work at a designated location.
• Performs other duties as assigned.
