Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative, and supportive care to help patients remain in their homes as long as possible.

The role requires an RN license in the state of employment, hospice or hospital experience preferred, EMR familiarity, a valid driver’s license, and BLS certification.

Qualifications

• Current license as RN in the state of employment.
• At least 1 year RN experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and auto insurance; reliable transportation.
• BLS for healthcare professionals required.

Responsibilities

• Provides direct care as part of the IDT and ensures holistic care for patients and families.
• Coordinates Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, prevention, and palliative options.
• Home visits to assess safety, medications, and living conditions.
• Communicates with IDT and physicians; SBAR documentation.
• Supervises Community Health Workers and Home Health Aides as requested.
• Reports to work during emergencies to ensure continuity of services.
• Performs other duties as assigned.
