Job summary

Heartland Hospice is seeking an experienced Registered Nurse Case Manager to lead patient care in home settings. You will perform comprehensive assessments, develop and update plans of care, and collaborate with an interdisciplinary team to deliver compassionate, patient-centered hospice services.

You will educate families, coordinate with physicians, and ensure timely documentation and compliance. This role requires 1+ year RN experience in hospice or relevant fields, CPR, and a state nursing

Qualifications

• RN with active nursing license in the state of employment
• Minimum 1 year nursing experience in acute care, hospice, home health or similar setting
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care
• Coordinate care with interdisciplinary team and physicians
• Educate patients and families on disease process, care at home and end-of-life decisions
• Maintain clinical documentation and ensure compliance with infection control standards
• Participate in on-call duties and patient support activities
