Job summary

Heartland Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to lead in-home hospice and palliative care. You will be the primary contact for patients and families, coordinating expert nursing care and ensuring plans of care are followed in patients' homes or other living settings.

You will perform comprehensive assessments, collaborate with physicians, social workers, and other team members, and provide education and support to families navigating end-of-life care.

Qualifications

• Current nursing license in the state of employment.
• CPR certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per individualized Plan of Care in home settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
