Job summary

Heartland Hospice is seeking a dedicated Registered Nurse Case Manager to lead in-home hospice care. You will assess patients, manage plans of care, and coordinate with an interdisciplinary team to deliver compassionate end-of-life care.

You will educate families, manage symptoms, and document care activities while ensuring compliance with regulatory standards and safety protocols in patients’ homes and alternative living environments.

Qualifications

• Minimum of 1 year RN experience in hospice, home health, or related setting.
• Current state RN license and CPR certification.
• Ability to collaborate with IDG and interdisciplinary teams.

Responsibilities

• Perform comprehensive patient assessments and educate families.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with physicians and nurse practitioners.
• Document patient records and ensure infection control compliance.
