Job summary

Heartland Hospice is seeking an experienced RN Case Manager to lead compassionate, patient-centered end-of-life care in home settings. You will assess, plan, implement, and document skilled nursing care aligned with the patient’s Plan of Care, collaborating with an interdisciplinary team to support patients and families.

Responsibilities include conducting comprehensive assessments, managing symptom control, educating families, and ensuring compliance with regulatory standards.

Qualifications

• 1+ year RN experience in hospice or related setting.
• Current RN license in state of employment.
• CPR certification.
• Strong assessment and communication skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care across interdisciplinary teams.
• Educate patient and caregiver on disease, self-care, and end-of-life needs.
• Communicate changes in condition to care team.
• Support continuity of care and transitions in levels of care.
• Document timely clinical records and ensure infection control compliance.
