Job summary

Heartland Hospice is seeking an experienced Registered Nurse Case Manager to lead in home-based hospice care. You will assess, plan, implement, and document nursing care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to build personalized plans.

Qualifications

• Active MO RN license in good standing
• Minimum 1 year RN experience in hospice, palliative care, or home health
• CPR certification required
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits
• Deliver skilled nursing interventions and symptom management per plan of care
• Develop and update individualized plans of care and visit frequency
• Coordinate care with interdisciplinary team and physicians
• Educate patients, families, and caregivers on disease, care options, and end-of-life processes
• Document clinical findings, orders, and care plans in a timely manner
• Communicate condition changes to the care team and ensure appropriate interventions
• Support continuity of care and transitions between levels of care
• Participate in on-call activities as applicable
