Heartland Hospice is seeking an experienced RN Case Manager to lead hospice care with a focus on home visits and personalized plans. You will coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive care.

The role emphasizes education, symptom management, and family support, with strong emphasis on documentation and regulatory compliance to uphold quality care.

Qualifications

• Active RN license in state of employment.
• Minimum 1 year of RN experience, hospice, home health, or palliative care preferred.
• CPR certification and willingness to perform in-home care.

Responsabilités

• Be the primary contact for patients and families guiding them through hospice care.
• Deliver and document skilled nursing care per the Plan of Care in home settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Educate caregivers and ensure safe home care practices.
