Job summary

Allina Health is seeking an experienced Hospice Case Manager (RN) to lead compassionate, patient- and family-centered end-of-life care. You’ll assess needs, plan and coordinate care, and work with a multidisciplinary team to ensure comfort and dignity, while supporting families through transitions.

This role emphasizes clinical leadership, collaboration, and adherence to standards; it includes on-call duties, weekend rotation, and a strong focus on evidence-based practice within the hospice

Qualifications

• RN with 2+ years of experience
• Licensed Registered Nurse in MN or WI upon hire
• BLS certification (American Heart Association or Red Cross) required

Responsibilities

• Comprehensive assessment of patient and family needs; identify outcomes based on needs assessment and establish plan of care.
• Supervise progress and modify the plan as needed; ensure high quality patient care through evidence-based practice.
• Coordinate, delegate, and collaborate with the care team to provide safe, effective care.
• Fulfills all organizational requirements and supports on-call and charge nurse roles as needed
