Job summary

Allina Health Hospice seeks a dedicated RN Case Manager to lead compassionate, personalized end‑of‑life care, helping patients live with dignity while supporting families through transitions. You will coordinate care with a multidisciplinary team and play a pivotal role in planning and evaluating patient needs.

The role offers 1.0 FTE (80 hours per two‑week pay period), 8‑hour day shifts, and every 4th weekend rotation, with on‑call as needed.

Qualifications

• RN degree and licensure to practice in MN/WI or state of hire.
• 2+ years RN experience required.
• Bachelor's degree preferred for advancement.
• Licenses/certifications: BLS required; CHPN preferred; CMGT-BC preferred.

Responsibilities

• Conduct comprehensive assessment of patient and family needs and develop a plan of care.
• Coordinate and delegate to ensure safe, effective care.
• Maintain competency and participate in continuing education.
• Collaborate with care team and family to achieve measurable outcomes.
• Support care continuity with on-call and weekend rotations.
