Job summary

Allina Health is seeking an experienced Hospice Case Manager to deliver compassionate, personalized home hospice care across patient visits. You will lead assessments, develop care plans, and coordinate with families and the care team to ensure comfort and dignity at life's transitions.

This role requires RN licensure and at least 2 years of nursing experience, with the ability to travel to patients' homes and work a day shift. On-call and weekend rotations may apply.

Qualifications

• Associate's or Vocational degree required.
• 2+ years RN experience.
• Licensed Registered Nurse in MN or WI depending on hire state.
• BLS certification (AHA or Red Cross) or in-house training.
• Driver's license required; travel to patients' homes.

Responsibilities

• Perform comprehensive assessments and establish care plans with measurable outcomes.
• Coordinate with family and care team to ensure safe, quality home hospice care.
• Provide in-home nursing care and maintain current competencies.
• Act as Charge Nurse or Preceptor when applicable.
