Qualifications

• RN licensure in good standing in Missouri.
• Hospice or end-of-life nursing knowledge required.
• Ability to assess patients, build care plans, and coordinate interdisciplinary care.
• Strong communication, documentation, and patient/family education skills.

Responsibilities

• Manage a caseload of hospice patients.
• Perform comprehensive patient assessments and develop/update individualized care plans.
• Coordinate care and services with an interdisciplinary team.
• Deliver expert pain and symptom management.
• Educate and support patients and families throughout the hospice journey.
• Ensure seamless communication with physicians and caregivers.
• Accurately document care and maintain compliance with clinical standards.
