Job summary

Allina Health is seeking a Hospice Case Manager in Minneapolis, MN. You will lead compassionate, patient-focused care, coordinate with the care team, and support families through transitions with dignity and comfort.

The role includes 1.0 FTE, 8-hour day shifts, and every 4th weekend rotation, with occasional on-call. RN with 2+ years of experience and MN/WI RN licensure per hire are required.

Qualifications

• RN experience (2+ years) required
• Associate's or Vocational degree in nursing
• Licensure as registered nurse required upon hire in MN/WI, per state
• BLS/CPR certifications preferred

Responsibilities

• Comprehensive assessment of patient and family needs; establish care plan and outcomes.
• Coordinate care with the interprofessional team to ensure safe, effective nursing care.
• Monitor progress, revise the plan of care as needed, and ensure high-quality patient care.
• Foster collaboration, delegate tasks appropriately, and fulfill organizational requirements.
