Allina Health Hospice is seeking a dedicated Hospice Case Manager in Minneapolis to provide compassionate, patient-centered end-of-life care. You will lead care planning, coordinate interdisciplinary teams, and support families through difficult transitions.

The role emphasizes clinical excellence, communication, and compassionate presence in a hospice setting. 1.0 FTE with 8-hour day shifts, 40 hours per week, and every 4th weekend rotation.

Qualifications

• Associate's or vocational degree required.
• 2+ years RN experience required.
• Nursing assessment and plan of care responsibilities.

Responsibilities

• Comprehensive assessment of patient and family needs and outcomes.
• Maintain education and competencies for high quality patient care.
• Incorporate evidence-based nursing practice considering regulations.
• Coordinate and delegate to ensure safe, effective care.
• Supervise plan of care and adjust as needed.
• Act as Charge Nurse or Preceptor when in those roles.
