Job summary

Allina Health seeks an experienced Hospice Case Manager to lead compassionate, personalized hospice care across assigned territory. You will assess patients and families, develop and coordinate a client-focused plan of care, and collaborate with the care team to ensure high-quality, compassionate services.

The role requires 1.0 FTE (80 hours per two-week pay period), 8-hour day/evening shifts, and every 4th weekend. Travel within the territory is expected with mileage reimbursement available.

Qualifications

• Associate’s or Vocational degree required.
• Minimum 2 years RN experience.
• RN licensure in MN/WI as applicable upon hire or state requirements.

Responsibilities

• Comprehensive assessment of patient and family needs; establish plan of care; supervise progress; modify plan as needed.
• Maintain education and competencies to provide high quality patient care.
• Coordinate, delegate, and collaborate to ensure care is safe and effective.
• Supplemental roles: Charge Nurse and Preceptor when applicable.
• Other duties as assigned.
