Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for Elite Hospice to oversee the Plan of Care for hospice patients in a home care setting. You will evaluate performance, develop care plans in collaboration with other professionals, and implement strategies that support physical, psychosocial, emotional, and spiritual well-being.

Experience with end-of-life care is preferred. Requirements include active RN licensure, a valid driver’s license with access to reliable transportation,

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG and attending physician.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family and care team to provide daily patient and family care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
