Cabrillo Hospice is seeking an experienced Hospice Registered Nurse Case Manager in Central LA. You will lead a multidisciplinary team, oversee care plans, and ensure regulatory compliance while delivering compassionate end-of-life care during home visits.

Are you an experienced Hospice RN who thrives in leadership and is passionate about providing high-quality end-of-life care? We are seeking a Hospice Registered Nurse Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. This role is ideal for a skilled leader who excels in case management, team coordination, and regulatory compliance, all while delivering compassionate care to patients and families.
The role is based in Central Los Angeles and requires the ability to travel multiple locations each day, typically conducting 1-10 home visits.
Be prepared to travel several hours per day, covering multiple locations, with 1-10 home visits daily.
Travel areas may vary and are subject to change based on patient needs.
Deliver compassionate, hands‑on care in diverse and unpredictable home environments.
Work in varied weather conditions, including rain, snow, and ice, while navigating stairs, different housing structures, and unique living conditions.
Utilize a tablet or laptop for real‑time medical record documentation during each patient visit.
Apply clinical expertise to make sound decisions that impact patient care.
Solve moderately complex problems using experience, precedent, and sound judgment.
Collaborate effectively with interdisciplinary team members to improve patient outcomes.
Extensive hospice and palliative care nursing knowledge.
Ability to manage patient needs and interdisciplinary team functions.
Proficiency in Microsoft Word, Excel, and EMR systems.
Strong interpersonal skills to communicate with patients, families, and caregivers in a compassionate and professional manner.
Sensitivity to patient privacy, HIPAA compliance, and ethical care practices.
Ability to adapt to the unique needs of each patient and family, ensuring they feel supported and valued.
Bachelor’s degree or higher in a health‑related field.
Minimum of 3 years of clinical RN experience, with hospice experience required.
At least 1 year of supervisory or management experience in a hospice setting.
Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards. JCAHO knowledge preferred.
Proficiency with Outlook, Word, and Excel.
Valid RN license in the state of practice.
Certified Hospice and Palliative Care Nurse certification preferred.
Active BLS certification through the American Heart Association or Red Cross.
Valid driver’s license and auto liability insurance for home visits.
