Laws & Associates is seeking a Hospice RN Case Manager to join our North Suburbs program serving Northbrook, Elmhurst, Wheeling, IL. This is a full‑time role focused on compassionate, patient‑centered hospice care in home settings.

You will coordinate with physicians and the interdisciplinary team, manage patient care, support families, and provide leadership within the care team. Requires 1+ year hospice or home health experience and certifications: AHA BLS and ACLS.

Qualifications

• 1+ year of hospice or home health experience.
• Experience in case management or care coordination.
• Strong clinical judgment and ability to work independently.
• A passion for patient-centered, end-of-life care.
• Certifications: AHA BLS (required), AHA ACLS (required).
