Qualifications

• RN license in IL is required.
• Minimum of 1 year RN experience; hospice experience preferred.
• Strong assessment and problem-solving skills.
• Excellent time-management and organizational abilities.
• Effective communication with patients, families, and team.

Responsibilities

• Plan, coordinate, and deliver hospice care per individualized plans.
• Collaborate with patients, families, and IDG for seamless care.
• Conduct comprehensive assessments and develop care plans.
• Provide education to patients and families about end-of-life care.
• Document evaluations, visits, and care per standards.
• Monitor changes and adjust care plans with physician input.
• Supervise Hospice Aides and LPNs for quality service.
• Stay updated on hospice regulations and best practices.
• Offer dietary counseling as needed.
• Participate in IDG meetings and staff development.
